Job summary

Amedisys is seeking a Registered Nurse to join our Hospice Case Manager team on the 1st shift. This full-time role focuses on patient-centered care in home settings, performing assessments and coordinating plans of care with the interprofessional team.

You will manage on-call responsibilities, educate families, document progress, and ensure timely communication with the clinical manager. Valid RN license and CPR are required, along with a driver's license and reliable transportation.

Qualifications

• One year of nursing experience.
• Current RN license valid in state(s) of assignment.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Maintains individualized patient care records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families and caregivers appropriate care techniques.
• Regularly communicates with clinical manager and care team.
• On-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
