Job summary

Amedisys is seeking an RN Case Manager in a full-time, days shift to join a leading home health and hospice team. You will assess patients, plan care with the team, and coordinate referrals to hospice disciplines as needed.

Responsibilities include documenting progress, teaching families, and on-call coverage. Active RN license and CPR are required; drivers with reliable transportation are preferred.

Qualifications

• One year of nursing experience required.
• Current RN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain and symptom changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
