Job summary

Amedisys is seeking an RN case manager to support patients in their homes. This full-time daytime role requires coordinating with the care team, assessing patient needs, and documenting care plans to ensure high-quality home health services.

You will work with a dedicated team, manage referrals, and participate in on-call coverage as needed. Amedisys emphasizes flexible growth and patient-centered care in a supportive environment.

Qualifications

• One year of nursing experience.
• Current RN license, state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops and maintains individualized patient care progress records per center policies.
• Records pain or symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with the clinical manager and care team.
• Performs on-call responsibilities and on-call services as assigned.
• Other duties as assigned.
