Job summary

Eternal Faith Hospice in Georgia is seeking an RN Case Manager to oversee admissions and provide nursing care for patients at their homes. You will coordinate with the Interdisciplinary Group/Team, patients and families, and referring agencies to ensure comprehensive care.

This role requires strong clinical skills, documentation, communication with physicians, on-call duties, and adherence to hospice standards.

Qualifications

• Two years RN experience in a clinical care setting.
• One year full-time hospice experience.
• Current State license as a Registered Nurse and a valid driver's license.
• Graduate of an accredited nursing program with solid nursing skills.

Responsibilities

• Admissions processing and coordination of care for hospice patients.
• Develop and document individualized plan of care in alignment with assessments.
• Provide nursing care following infection control measures.
• Coordinate with Interdisciplinary Group/Team and referring agencies.
• Communicate with attending and hospice physicians and other care providers.
• Submit required paperwork in a timely manner.
• Participate in hospice-sponsored in-service and QAPI programs.
• Maintain productivity standards and participate in on-call coverage.
• Provide clear communication to patients, families, and health professionals.
• Travel to patient homes as needed and maintain valid auto liability insurance.
