The Hospice RN Case Manager is a full-time, on-site role based in Hinesville, GA, responsible for coordinating and delivering comprehensive nursing care to hospice patients in alignment with individualized care plans. Day-to-day responsibilities include conducting thorough patient assessments, managing pain and other symptoms, administering medications and treatments, and monitoring changes in condition. The RN Case Manager collaborates closely with physicians, social workers, chaplains, and other interdisciplinary team members to ensure continuity of care, appropriate utilization of services, and timely discharge planning when needed. This role also involves educating patients and families on disease processes, care techniques, and available resources, advocating for patient preferences, and maintaining accurate, timely documentation that adheres to regulatory and agency standards.
