Job summary

Houston Hospice is seeking a PRN - RN Field Case Manager to support our Southwest Territory in the Houston area. The role focuses on coordinating comprehensive home-based hospice care and ensuring high-quality symptom management for patients and families.

The ideal candidate has an active RN license and 2+ years of case management or hospice experience. You will collaborate with physicians, nurses, social workers, and other providers, and may conduct home visits as needed.

Qualifications

• Active RN license required.
• 2+ years of case management or hospice experience preferred.
• Ability to coordinate care with physicians, nurses and other providers.

Responsibilities

• Coordinate care with providers including RN and LPNs.
• Maintain case documentation in accordance with state regulations.
• Develop and maintain client care plans based on clinical assessment and family needs.
• Coordinate care with other providers as necessary.
• Conduct home visits as necessary to ensure optimal outcomes.
• Review patient charts weekly to ensure appropriate follow-up and referral as necessary.
• Ensure follow-up is provided by all other providers as necessary.
