Treasure Coast Hospice in Fort Pierce, FL is seeking a Registered Nurse Case Manager - Homes to plan and deliver skilled nursing care and oversee patient care under Florida Nurse Practice Act.

The role requires a current Florida RN license, CHPN within 12 months, CPR, a valid driver's license, and prior RN experience. A $4,500 sign-on bonus is offered; schedule is Mon-Fri, 8am-4:40pm, with field team responsibilities.

Formación

• Current and active Florida RN license.
• CHPN certification within 12 months.
• Valid Florida driver’s license and automobile insurance.
• Current CPR certification.
• Minimum of one year RN experience with assessment skills.
• Experience in oncology, home health, hospice or palliative care preferred.
• Ability to handle sensitive issues with professional boundaries and confidentiality.
• Self-directed and able to work independently in the field.
• Basic computer skills and EMR software experience preferred.
• Attendance is a condition of employment.
• Meet program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments.

Responsabilidades

• Plan and deliver skilled nursing care per policies and Florida Nurse Practice Act.
• Provide palliative and supportive care to patients and families.
• Supervise CNAs and LPNs.
• Credentialing information for area facilities and hospitals.
• Communicate with the IDT and document care plans.
• Maintain documentation of interventions and outcomes.
• Participate in compliance program and report as required.
• Support strategic plan and align with mission/values.

Conocimientos

Florida RN license
