Job summary

The VNA of the Treasure Coast is seeking a full-time Registered Nurse to oversee patient caseload in Indian River County Home Health. You will coordinate services with an interdisciplinary team, perform initial assessments, and develop and execute care plans with focused documentation.

You will supervise LPNs and HHAs, schedule visits, and collaborate with physicians to update plans of treatment. Florida RN license and home health experience required; Oasis and Home Base knowledge preferred.

Qualifications

• Current RN License in the State of Florida.
• Must have two years of home health experience.
• Must have working knowledge of Home Care Home Base - PointCare.
• Must have working knowledge of Oasis documentation.

Responsibilities

• Assess client environment and family support to develop and implement care plans.
• Perform admission assessments and establish treatment plans.
• Continuously evaluate patient response and revise care plan as needed.
• Provide direct nursing care and coordinate with interdisciplinary team.
• Coordinate all services and communicate with physicians about treatment plans.
• Maintain clinical records and complete documentation daily.
• Schedule client visits on a weekly basis and adjust as needed.
• supervise LPNs and HHAs in accordance with regulations.
