Job summary

Chapters Health System in Florida is seeking an RN Case Manager to assess and identify patient/family needs, coordinate Plan of Care with the IDT, and provide clinical, palliative and supportive care to help patients remain at home.

This role involves home visits, education of patients and families, collaboration with physicians, and supervision of home health staff, with a Monday–Friday daytime schedule and mileage reimbursement.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Team (IDT).
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient, family, caregivers and other health professionals about disease process and palliative care.
• Home visits to assess home safety, medication and nutritional compliance.
• Communicates changes in patient’s condition to IDT and physicians.
• Participates with the IDT to evaluate hospice referrals/admissions for level of care.
• Attends IDT meetings and presents concise reports; collaborates with all disciplines.
• Supervises patient care by Home Health Aides and CHWs as requested.
