By The Bay Health in Sonoma County seeks a Registered Nurse to serve as Hospice Nurse Case Manager. This full‑time role focuses on coordinating home-based care, collaborating with an interdisciplinary team, and guiding patients and families through care planning.

Schedule is Monday through Friday, 8:30am to 5:00pm, with 3-5 visits per day within a narrow geographic area. Requires California RN license; Public Health Nursing certification desirable; HPNA preferred.

Qualifications

• AA or BSN from an accredited nursing program.
• Experience in acute care/home health/hospice preferred but not required.

Responsibilities

• Provide in-home nursing care as part of the interdisciplinary team.
• Coordinate patient/family services and implement individualized care plans.
