Optimal Care in Mount Pleasant, MI is seeking a Hospice RN Case Manager. You will provide skilled nursing care and coordinate comprehensive home-based care focused on comfort and quality of life for patients and families.

In this role you will build rapports with patients, families, physicians, and facilities while collaborating with an interdisciplinary team. Travel within the service area is required.

Formación

• Graduate of an accredited nursing program and current RN license.
• Minimum one year nursing experience in a clinical setting.
• Hospice or home health experience preferred.

Responsabilidades

• Provide skilled nursing care and compassionate support to patients and families in their homes or facilities.
• Coordinate comprehensive care with an Interdisciplinary Team across service areas.
• Build report with patients, families, physicians and facilities, ensuring quality of life.
• Travel daily within the service area to deliver in-home care.
