Job summary

AdvisaCare is seeking a dedicated Hospice RN Case Manager for full-time work in Michigan's Mount Pleasant area, serving the Bay City/Midland/Saginaw Tri-City region with compassionate at-home hospice care.

You will perform patient assessments, develop and revise plans of care, supervise paraprofessionals, document progress, and participate in interdisciplinary team meetings to improve quality and patient comfort.

Qualifications

• Active Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and families.

Responsibilities

• Initial and ongoing patient assessments in home settings.
• Implement and revise the plan of care with the interdisciplinary team.
• Supervise paraprofessionals and document progress.
• Participate in IDG conferences and performance-improvement activities.
• Maintain high satisfaction with patients, families, and caregivers.
