Job summary

Optimal Care in Southwest Michigan seeks a Registered Nurse to deliver skilled nursing care in home health and hospice settings. You will assess patients, develop care plans, and work with physicians, families, and an interdisciplinary team to support independence and comfort.

This high travel role covers Kalamazoo, St. Joseph, Allegan and surrounding counties. Full-time, part-time, or per diem options with generous benefits are available, including PTO, health insurance, and a 401(k) match.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• Minimum one year experience as a nurse in a clinical care setting.
• Reliable transportation, valid driver’s license, and automobile insurance.

Responsibilities

• Manage a caseload of patients in home health or hospice settings.
• Conduct comprehensive patient assessments and develop care plans.
• Coordinate services with physicians, families, facilities and interdisciplinary team.
• Deliver skilled nursing care with clinical independence and sound judgment.
• Maintain documentation and ensure compliance with regulations.
