Job summary

Optimal Care is seeking a Hospice RN Case Manager to deliver skilled nursing care and compassionate support in patients' homes and facilities across Clare, MI area. This role emphasizes building deep relationships with patients, families, physicians, and the interdisciplinary team while coordinating comprehensive care.

You will travel within the service area to provide high-quality end-of-life care, with a Monday–Friday schedule and weekend rotation.

Qualifications

• Graduate of an accredited Nursing program (Diploma/Associate/Bachelor).
• Current State RN license in good standing.
• Minimum 1 year nursing experience in a clinical setting.
• Reliable transportation and valid driver's license.

Responsibilities

• Coordinate and provide skilled nursing care for a caseload in home settings.
• Collaborate with interdisciplinary team to meet physical, emotional, and spiritual needs.
• Establish rapport with patients and families; manage documentation and timelines.
• Travel daily within service area to patient homes, facilities or nursing homes.
