Job summary

Ennoble Care is seeking a Hospice Registered Nurse Case Manager to service patients in Middlesex/Somerset County, NJ. The RN will work with an interdisciplinary team to provide skilled nursing assessments, planning, and care to maximize patient comfort and support families.

The role includes caseload management, collaboration with physicians and Hospice staff, and thorough documentation in EMR. Candidates must be NJ-licensed RNs with hospice experience preferred and a valid driver’s license.

Qualifications

Must be a graduate from an accredited School of Nursing.
Must be licensed in NJ as an RN by New Jersey State Board of Nursing.
One year of RN experience; hospice experience preferred.
Excellent assessment and problem-solving skills.
Strong organization/time-management abilities; knowledge of case management processes.
Must be a licensed driver with a reliable insured vehicle.
Effective verbal and written communication skills.

Responsibilities

Manage a patient caseload; assess, plan, implement and evaluate the nursing process.
Coordinate care with attending physicians, Hospice staff and other agencies.
Maintain timely documentation in electronic medical records (EMR).
Contribute to Interdisciplinary Group meetings and share information.
Provide caregiver education and coordination of on-call coverage.
Support patients and families during bereavement and at time of death.
Lead patient care conferences and assist with staff orientation and training.
