Job summary

Gentiva Hospice in Houston, TX seeks a dedicated RN Case Manager to lead hospice care at patients' homes and other settings. You will coordinate, assess, educate, and support patients and families, delivering compassionate end-of-life care with a multidisciplinary team.

You’ll perform comprehensive assessments, develop care plans, manage symptoms, and ensure timely documentation. This role emphasizes empathy, clinical excellence, and regulatory compliance in a dynamic home health environment.

Qualifications

• Registered Nurse with active license in the state of employment.
• Minimum of 1 year RN experience in acute care, oncology, long-term care, hospice or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients and caregivers on disease, end-of-life care, medications and nutrition.
• Document timely clinical records and ensure regulatory compliance.
• Support continuity of care and transitions in levels of care.
