Job summary

Dierksen Hospice in Texarkana, AR seeks a Full-time Hospice RN Case Manager to join our caring team, delivering hands-on hospice nursing, coordinating services, and guiding patients and families through their hospice journey.

As part of our interdisciplinary team, you will work Monday–Friday with a 60-mile territory radius and be supported by a comprehensive benefits package designed to promote well-being and professional growth.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical surgical/acute care setting; hospice experience a plus.
• Must be computer proficient with EMR experience.
• CPR certification as required by state.
• Valid driver's license, vehicle insurance, and dependable transportation.

Responsibilities

• Empower Through Engagement: Respond to clinical referrals with care and expertise.
• Collaborative Care Champion: Work with Clinical Director to assess client eligibility and needs.
• Nurturing Direct Care: Provide RN case management focusing on comfort and quality of life.
• Nursing with Heart: Deliver care using proven nursing principles to ease pain and provide solace.
• Educator and Advocate: Educate families and involve them in personalized care plans.
• Call of Compassion: Shared call duties reflecting commitment to being there when it matters.
