Job summary

Four Seasons is seeking an RN Case Manager to coordinate nursing care, supervise staff, and ensure regulatory compliance for a caseload of approximately 15 patients in Buncombe County and surrounding areas. The role requires strong clinical skills, patient education, and collaboration with the Clinical Team Leader.

The position is full-time with a standard 40-hour week (Mon-Fri, 8:30am-4:30pm) and a focus on delivering high-quality hospice nursing care in a supportive team environment.

Qualifications

• Coordinate nursing care and supervise LPN/Hospice staff.
• Knowledge of HIPAA and healthcare regulations.
• Maintain nursing plans of care and patient documentation.

Responsibilities

• Coordinate nursing plan of care for ~15 patients.
• Provide direct nursing care and education to patients/families.
• Supervise staff and ensure compliance with regulatory standards.
