Job summary

Compassus in Prescott, AZ is seeking a Hospice Registered Nurse / RN Case Manager to provide high-quality home-based hospice care. You will develop care plans, coordinate with the care team, and support patients and families in meaningful ways while upholding the Compassus mission.

The role requires an active RN license, 2+ years of nursing experience (hospice preferred), and CPR/BCLS. A valid driver’s license is required to serve clients at home across the service area.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, hospice experience preferred
• Current driver’s license
• Current CPR and BCLS certification
