Job summary

Lighthouse Hospice NJ is seeking a full-time Hospice Registered Nurse Case Manager (RNCM) to provide compassionate end-of-life care across Mercer, Burlington, Camden, Gloucester and Salem counties. The role focuses on coordinating services and guiding patients and families through the hospice journey.

Schedule is Monday–Friday, 8 am–4:30 pm. We offer a strong benefits package and opportunities for growth within an interdisciplinary team dedicated to dignity and comfort at every life moment.

Qualifications

• Active RN license (unencumbered).
• Minimum 1 year RN experience in med-surg/acute care; hospice a plus.
• Computer proficiency including EMR.
• CPR certification (if required by state).
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Partner with Clinical Director to assess clients for hospice services.
• Provide direct nursing care to support comfort and quality of life.
• Deliver nursing care grounded in proven principles and techniques.
• Educate and advocate for patients and families to support personalized care plans.
• Participate in call duties and be there when it matters most.
