Job summary

Stillwater Hospice, a growing end-of-life care provider, is interviewing for a Hospice RN Case Manager in Billings, MT. You will lead nursing care, assess patients and coordinate services with the interdisciplinary team to support patients and families at home.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of acute care in an institutional setting and 1–2 years in home care.
• Current licensure in the state.
• Licensed driver with insured vehicle.
• Bachelor’s degree; hospice care experience preferred.
• Management experience not required.
• Excellent communication, problem solving and basic math skills.

Responsibilities

• Conduct initial and ongoing patient assessments to determine hospice needs.
• Develop and revise the plan of care with the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Counsel patients and families on nursing needs and self-care.
• Coordinate patient care as Case Manager for assigned caseload.
• Maintain accurate clinical notes and communicate with physicians.
• Participate in on-call duties and care coordination.
• Supervise hospice aides when required.
