Qualifications

• Bachelor’s in social work CSWE-accredited required.
• Master of Social Work preferred.
• Minimum one year healthcare experience; hospice knowledge preferred.
• Licensed and insured automobile required.

Responsibilities

• Assess psychosocial status of patients and families; inform care team.
• Conduct social evaluations, plan interventions, and provide crisis support.
• Educate the care team on cultural needs and family dynamics.
• Maintain clinical records and progress notes for patients.
• Assist in preparing advanced directives and referrals.
• Coordinate with community agencies and support services.
