Hospice of North Alabama seeks an RN Case Manager to provide comprehensive hospice care in the Hartsel le, Eva, and Laceys Spring area from Cullman. The role covers Morgan County with a Monday–Friday schedule and on-call rotation, delivering patient-centered nursing care and care planning that aligns with patient goals.

The ideal candidate holds an accredited nursing degree, state licensure, and 1 year of nursing experience with hospice experience preferred; valid driver and insurance are

Qualifications

• Graduate of an accredited nursing school with current state licensure
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with a valid automobile and insurance

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with patient wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
