Job summary

Hospice of North Alabama is seeking a dedicated RN Case Manager to join our team in Cullman, AL. You will assess patients, develop individualized hospice care plans, administer medications, and coordinate with an interdisciplinary team to ensure comfort and quality of life for patients and families.

Excellent clinical and documentation skills are essential, with 1+ year nursing experience (3+ years preferred) in hospice or similar settings.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred ( hospice or similar setting preferred).
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
