Job summary

Hospice of North Alabama in Cullman, AL is seeking a Registered Nurse to join our hospice team as an RN Case Manager. You will assess patient needs, develop care plans, provide direct nursing care and symptom management, and support families throughout the end-of-life journey.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
