Hospice of North Alabama is seeking a dedicated RN Case Manager to deliver patient-centered hospice care in the Florence, AL area. The role emphasizes assessments, care plan development, symptom management, and family education within an interdisciplinary team.

Ideal candidates hold an active nursing license, CPR certs, and prefer applicants with hospice experience. The position offers full-time hours with stable employment and opportunities for growth.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured automobile.
• Maintain CPR certification where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, interventions.
• Provide pain and symptom management.
• Educate and support patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required.
