Embrace Hospice in Kerrville, TX is seeking a dedicated Hospice RN Case Manager to provide compassionate, patient-centered home care within a 50‑mile radius. You will assess needs and develop personalized care plans for patients at a critical time.

Responsibilities include direct nursing care, pain and symptom management, family education, interdisciplinary team collaboration, precise documentation, and on-call rotations.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Must be a licensed driver with a vehicle in good standing

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with patient wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
