Job summary

Embrace Hospice in Kerrville, TX is seeking a dedicated RN Case Manager to provide 1:1 patient care and develop individualized hospice plans. You will work with a supportive interdisciplinary team to manage symptoms, educate families, and ensure quality documentation.

The role requires nursing licensure in Texas, at least 1 year of nursing experience (3+ years preferred) and a driver with insured vehicle. Benefits include comprehensive insurance, retirement plans, and paid time off.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Education

Nursing degree / RN licensure

One year nursing experience required

3+ years hospice experience preferred

Knowledge of hospice philosophy of care

Commitment to clinical and documentation excellence
