Job summary

Millenia Hospice is seeking a compassionate PRN Hospice Case Manager to join our team in the Dallas–Fort Worth area. You will conduct thorough patient assessments, develop personalized care plans, and provide skilled nursing care and symptom management in collaboration with an interdisciplinary team.

You will educate families, monitor patient progress, adjust plans as needed, and maintain compliant clinical documentation.

Qualifications

• Current and unrestricted RN license in Texas
• Minimum 1–2 years hospice/home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively
• Reliable transportation and valid driver’s license
• Current CPR certification is required

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing care and symptom management
• Coordinate care with physicians, social workers, chaplains, and other team members
• Educate patients and families on disease progression, medications, and care techniques
• Monitor patient condition and adjust care plans as needed
• Maintain accurate and timely clinical documentation
• Participate in on‑call rotation as needed
• Ensure compliance with hospice regulations and standards
