Job summary

Patriot Healthcare in Anderson, SC is seeking an experienced RN Case Manager to coordinate hospice services with an Interdisciplinary Team. You will apply the nursing process to develop and implement patient plans of care, ensuring dignity and comfort at life’s end.

The role requires an SC RN license and a nursing degree; hospice experience is preferred. This full-time position offers flexible scheduling, competitive pay, and comprehensive benefits through Patriot Healthcare.

Qualifications

Must possess a Nursing Degree from an accredited college or university.
Must be licensed as a Registered Nurse in the State of South Carolina.
1 year of experience as a nurse in a hospital, long-term care facility, or other related health care facility is preferred.
Hospice experience also preferred.

Responsibilities

Coordinate and direct the provision of hospice services according to physician orders and patient plan of treatment based on hospice policies and procedures.
Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
Follow the Nursing Standards of PH Healthcare.
Demonstrate familiarity with hospice policies, state and federal regulations pertaining to hospice and nursing services.
Implement hospice nursing care in accordance with current rules, regulations, and guidelines.
Make written and oral reports/recommendations to the Director of Clinical Services as necessary.
