Job summary

Adoration Health is seeking a Registered Nurse Case Manager for hospice to provide patient-centered care in the Fayetteville/Peachtree City/Senoia area. The role focuses on assessing patient needs, creating individualized care plans, delivering direct nursing care, and supporting families throughout the care journey.

You will collaborate with an interdisciplinary team, maintain thorough documentation, and participate in on-call rotations.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
