Job summary

Adoration Health is seeking a dedicated Hospice RN Case Manager in McDonough, GA to join our team. You will assess and address the physical, emotional, and psychological needs of patients, and create individualized hospice care plans while delivering direct nursing care including medications and treatments.

The role emphasizes pain and symptom management, family education, collaboration with an interdisciplinary team, and meticulous documentation.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Must be a licensed driver with insured automobile in accordance with state requirements

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
