Job summary

Adoration Health is seeking a Registered Nurse Case Manager for Hospice services in the Fayetteville area. This full-time role supports patient-centered, at-home care with a dedicated team and opportunities for professional growth.

The RN will assess patient needs, develop care plans, administer treatments, and educate families. A valid nursing license, relevant experience in hospice, and a driver’s license are required.

Qualifications

• Graduate of an accredited nursing school with current state licensure.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a valid automobile.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
