Job summary

Adoration Health in Newnan, GA is seeking a compassionate RN Case Manager to join Adoration Hospice. You will provide patient-centered, 1:1 care in a home setting, aligning care plans with patient wishes and family goals.

You will assess physical, emotional, and psychological needs, administer medications and treatments, and educate families. You will collaborate with an interdisciplinary team and participate in on-call rotations as required by the local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align the patient’s wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
