Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Hospice RN Case Manager to provide patient-centered care in the McDonough, GA area. This role focuses on comprehensive hospice planning, collaboration with a multidisciplinary team, and hands-on nursing care delivery in a home/hospice setting.

The ideal candidate will have RN licensure, 1 year of nursing experience (3+ years preferred), and a commitment to clinical excellence and compassionate support for patients and

Qualifications

• Graduate of an accredited nursing school with current RN license in state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a valid driver''s license and an automobile in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient''s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
