Adoration Health in Newnan, GA is seeking a compassionate Registered Nurse Case Manager to deliver expert hospice care across Fayetteville, Peachtree City and Senoia. This full-time RN will coordinate patient care, plan care aligned with patient goals, administer medications, and support families.

You will join a supportive team with on-call rotation, opportunities for growth, and a stable schedule in a growing home health organization.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications
