Job summary

Springhill Hospice in Daphne, AL seeks an experienced RN Case Manager to join our hospice team. You will assess and monitor patients, create individualized care plans, and provide direct nursing care including medications and symptom management.

The role emphasizes collaboration with an interdisciplinary team, educating families, and maintaining precise documentation. A current Alabama RN license and CPR certification are required, with on-call rotation as needed.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with a vehicle in good working order
• Possess and maintain CPR certification where required

Responsibilities

• Assess, monitor physical, emotional, and psychological needs of patients
• Create hospice care plans aligned with patient wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Education
