Job summary

Hospice of North Alabama in Decatur, AL seeks an experienced Registered Nurse Case Manager to provide compassionate, patient-centered care in the home setting. You will develop and implement individualized care plans and administer medications as part of a dedicated interdisciplinary team.

You will collaborate with families and caregivers, maintain thorough documentation, and participate in on-call rotation to ensure seamless hospice services for patients in Morgan County, including Hartselle

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured and in good working order
• Experience in hospice or a similar setting preferred
• One year nursing experience required, 3+ years preferred

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Education

Nursing degree (RN) with state licensure
