Job summary

Springhill Hospice is seeking an experienced RN Case Manager in Mobile, AL to provide patient-centered hospice care. You will assess patients, develop and implement care plans, and collaborate with an interdisciplinary team to ensure comfort and quality of life.

The role emphasizes compassionate, autonomous nursing with strong documentation and coordination of care for patients and families in the Mobile area.

Qualifications

• Graduate of an accredited nursing program with active RN licensure in Alabama.
• Minimum 1 year of nursing experience required; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care and strong documentation practices.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create and implement hospice care plans aligned with patient goals.
• Direct nursing care including medications, treatments, and interventions.
• Provide pain/symptom management and education to families/caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by local branch.
