Job summary

Hospice of the Midwest in Johnston, IA is seeking a dedicated Hospice RN Case Manager to provide patient-centered, compassionate care at a critical time. The role requires assessing physical, emotional, and psychosocial needs, creating individualized care plans, and delivering direct nursing care including medications and interventions.

You will educate families, collaborate with an interdisciplinary team, maintain documentation, and participate in on-call rotations.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
