Job summary

Comfort Hospice Care in Nevada seeks an experienced Registered Nurse to join as a Hospice RN Case Manager. You will assess patients and families, develop care plans, and deliver skilled nursing care in home settings, guiding families through decisions with compassion.

Educate families on care techniques and medications, maintain documentation, and participate in team meetings. A supportive, growth-focused environment with ongoing education awaits.

Qualifications

• Currently licensed as a Registered Nurse in Nevada.
• Minimum of 2 years of nursing experience, hospice or home health preferred.
• Homecare Homebase (HCHB) experience preferred, not required.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Develop and implement individualized care plans and revise as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Supervise hospice aides and delegate responsibilities appropriately.
