Job summary

Samaritan in New Jersey is seeking a Hospice Nurse to oversee the quality and expert care of assigned hospice patients in a full-time role. You will perform patient assessments for comfort and symptom relief, establish individualized plans of care, educate patients and families, obtain physician orders, coordinate home visit schedules, and collaborate with social workers and IDT members as needed.

• Minimum of two years of nursing experience.
• NJ RN license required; BSN preferred.
• Bilingual is a plus.

What You’ll Do:

• Triage and arrange home visit schedule, demonstrating efficiency in time and mile management.
• Make referrals to additional IDT members and collaborate with social workers related to their involvement.
