Job summary

HopeHealth Hospice & Palliative Care is seeking an experienced RN Case Manager to oversee hospice patients in LTC facilities in southern Rhode Island. The role requires advocacy, coordination of care, and timely documentation. This full-time position offers a daytime schedule with a rotating weekend compensation.

The RN will assess patients, develop and monitor plans of care with physicians, and provide education to patients and families to ensure high-quality end-of-life care.

Qualifications

• RN license required; RI (Massachusetts preferred) license welcome.
• BSN preferred by the employer.
• Minimum 1–3 years of clinical experience.
• Excellent assessment, clinical and communication skills.

Responsibilities

• Conducts initial and ongoing assessment of patients.
• Collaborates with physician to manage patient's plan of care.
• Evaluates plan of care to meet changing patient needs.
• Provides ongoing training and education to patients and family.
• Initiates preventive and rehabilitative nursing procedures and referrals.
• Prepares and submits timely patient documentation.
• Coordinates services and informs staff of changes in patient condition.
• Determines scope and frequency of services based on acuity.
• Maintains accurate documentation of patient care.
