Titan Placement Group is seeking a Hospice Case Manager (RN) in Warwick, RI to provide compassionate end-of-life care in homes and facilities. You will manage your own patient caseload and work with an experienced interdisciplinary team to deliver exceptional hospice services.

Qualifications

• Active Rhode Island RN license or Compact RN license eligibility for Rhode Island.
• At least 1 year of nursing experience.
• Hospice or home health experience preferred.
• Value compassionate, patient-centered care and ability to work independently with an interdisciplinary team.
