Gentiva Hospice in Rhode Island is seeking an RN Case Manager to provide compassionate, home-based end-of-life care. You will assess patients, develop and update care plans, and work with an interdisciplinary team to ensure comfort and dignity for patients and families.

You will educate families, document care, coordinate with physicians and support staff, and participate in on-call shifts as required. An active Rhode Island RN license and at least 1 year of hospice/home health/acute care

Formación

• Active RN license in Rhode Island
• Minimum 1 year RN experience in hospice, home health, or acute care
• Ability to develop and update individualized care plans

Responsabilidades

• Perform initial and ongoing patient assessments and documentation
• Coordinate with interdisciplinary team to implement plans of care
• Educate patients and families on disease, care options, and bereavement support
• Deliver skilled nursing interventions and symptom management
• Maintain accurate clinical records and comply with infection control standards
