Job summary

Interim HealthCare of West Michigan is seeking a Registered Nurse for our Hospice division. The role is flexible and essential, with strong support from management and a focus on advocating for patients.

You will complete patient assessments, collaborate with the IDG to plan care, educate patients and families, ensure availability of equipment and medications, and document services accurately while attending IDG meetings as needed.

Qualifications

• Active RN license in Michigan (unrestricted).
• At least 1 year of RN experience; home health preferred.
• CPR certification required or maintained.

Responsibilities

• Complete ongoing assessments of patient physical and psycho-social status.
• Develop and revise the care plan with IDG, patient, family, and physician.
• Provide education and interventions for pain and symptoms; ensure equipment and supplies are available.
