Job summary

St. Mary's Home Care and Hospice, part of Trinity Health At Home, in Athens, GA, is seeking a Contingent Hospice RN Case Manager to provide professional nursing care to hospice clients at home, in line with physician orders and regulatory standards.

The RN Case Manager will assess and plan care, collaborate with an interdisciplinary team, and coordinate with the physician and hospice medical director. Position is contingent, primarily Monday–Friday with occasional weekend coverage.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Georgia.
• One (1) year experience as a professional acute care nurse.
• Home care experience preferred but not required.

Responsibilities

• Provide hospice nursing care to clients in home settings as prescribed.
• Assess, plan, implement, and supervise patient care and outcomes.
• Collaborate with interdisciplinary team and attend weekly meetings.
• Coordinate with primary care physician and hospice medical director as needed.
• Provide support to Hospice Aides and LPN/LVNs as needed.
