Job summary

St. Mary’s Home Care and Hospice, part of Trinity Health at Home, seeks a Contingent Hospice RN Case Manager in Athens, GA. You will provide nursing care in clients’ homes, manage assessments, care plans, and outcomes while coordinating with physicians and the hospice team.

Minimum qualifications include GA RN licensure and at least 1 year of acute care experience; home care experience is preferred. Schedule is primarily Monday–Friday, with occasional weekend coverage; mileage reimbursement and

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Georgia.
• One (1) year experience as a professional acute care nurse.
• Home care experience preferred but not required.

Responsibilities

• Provide professional nursing care to the organization’s hospice clients as prescribed by the physician.
• Assess, plan, intervene, and supervise patient care and outcomes.
• Collaborate with interdisciplinary team and attend weekly meetings.
• Territory: Athens, GA; contingent Mon-Fri with occasional weekend coverage.
