Job summary

St. Mary's Home Care and Hospice in Athens, GA is seeking a Contingent Hospice RN Case Manager to provide compassionate nursing care to hospice clients in home settings. The RN Case Manager will lead assessments, care planning, and symptom management, coordinating with physicians and the interdisciplinary team.

The role emphasizes patient-centered care, collaboration with the care team, and flexible scheduling as part-time, contingent coverage.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Georgia.
• One (1) year experience as a professional acute care nurse.
• Home care experience preferred but not required.

Responsibilities

• Provide nursing care to hospice clients as prescribed by physician and policy.
• Assess, plan, implement, and supervise care and outcomes.
• Collaborate with interdisciplinary team and attend weekly meetings.
• Interact with primary care physician and hospice medical director as needed.
• Support Hospice Aides and LPN/LVN as needed.
