Job summary

St. Mary's Home Care and Hospice is seeking a contingent Hospice RN Case Manager to deliver professional nursing care to hospice patients in their homes in Athens, GA.

The RN will assess, plan, and oversee care, coordinating with the physician and a multidisciplinary team to achieve patient-centered outcomes. The role requires Georgia RN licensure, a nursing degree, and at least one year of acute care experience; home care experience is preferred.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Georgia.
• One year of professional acute care nursing experience; home care preferred.

Responsibilities

• Provide one-to-one care with patients in their homes.
• Collaborate with interdisciplinary hospice team and participate in weekly meetings.
• Manage patient symptoms and implement the hospice plan of care.
• Support families with grief/loss and coordinate with physicians.
